APPROVE and AUTHORIZE the Health Services Director, or designee, to execute, on
behalf of the County, Contract Extension Agreement #74-344-3 with The Regents of the
University of California, San Francisco, an educational institution, effective January 31,
2012, with no change to the original Payment Limit of $8,080 and to extend the term from
January 31, 2012 through January 31, 2013.
FISCAL IMPACT:
None, there is no change in the Contract Payment Limit of $8,080. This Contract is funded
100% by the Mental Health Services Act (MHSA). (No rate increase)
BACKGROUND:
On July 7, 2009, the Board of Supervisors approved Contract #74-344 (as amended by
Extension Agreement #74-344-1 and #74-344-2), with The Regents of the University of
California, San Francisco, for the provision of clinical evaluation and consultation services
to the County’s Mental Health Division staff with regard to providing an overview of the
University of

BACKGROUND: (CONT'D)
Washington’s Depression Care Model (IMPACT Model) and certifying selected County
clinical staff with regard to Problem Solving Therapy (PST), including mutual
indemnification, through January 31, 2012.
Approval of Contract Extension Agreement #74-344-3 will allow the Contractor to continue
providing services, including mutual indemnification for any claims that arise out of the
performance of this Contract, through January 31, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this extension is not approved, Contractor will not continue to provide services.
