south-facing hangar at Buchanan Field Airport effective November 17, 2021 in
the monthly amount of $350, Pacheco area (District IV).
C. 6 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a
month-to-month hangar rental agreement with Michael McCarthy, for a
north-facing shade hangar at Buchanan Field Airport effective November 3, 2021
in the monthly amount of $165, Pacheco area (Airport Enterprise Fund 100%).
C. 7 APPROVE and AUTHORIZE the Director of Airports, or designee, to execute a
month-to-month hangar rental agreement with Bright Apps, LLC. for a

APPROVE and AUTHORIZE execution of agreements between the County and the
following agencies for receipt of fund and/or services:

designee, on behalf of its Aging and Adult Services Bureau, to apply for and
accept funding from the California Department of Health and Human Services,
Housing Bureau in an amount not to exceed $1,382,181 for the period of July 1,
2021 through June 30, 2024. (100% State)
C. 46 APPROVE and AUTHORIZE the County Librarian, or designee, to apply for and
accept a grant in the amount of $75,000 from Sunlight Giving, a private nonprofit
corporation, to provide early literacy interactive panels and kiosks to libraries in
impacted communities in Contra Costa County, for the period November 1, 2021
through June 30, 2022. (44% County match, Library Fund)
C. 47 APPROVE and AUTHORIZE the Auditor-Controller, or designee, to pay
stipends to seven contracted childcare partner agencies in an amount not to exceed
$244,200 ($600 per child enrolled in March 2021) pursuant to California

C. 48 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract amendment with the City of Pleasant Hill, to increase the

in the amount of $492,057 for a new total amount of $6,478,915 with no change to

Health Centers for the period August 11, 2021 through August 10, 2026. (100%

C. 52 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Positive Pathways, LLC, in an amount not to exceed
$4,500,000 to provide applied behavior analysis services for Contra Costa Health
Plan members for the period October 1, 2021 through September 30, 2024. (100%

C. 53 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with Herbert A. Holman, M.D., in an amount not to exceed
$300,000 to provide dermatology services to Contra Costa Health Plan members
and County recipients for the period December 1, 2021 through November 30,

C. 54 APPROVE and AUTHORIZE the Public Works Director, or designee, to execute
a contract amendment with Fennie + Mehl Architects, effective October 17,
2021, to increase the payment limit by $50,000 to a new payment limit of
$350,000, to extend the term from October 17, 2021 to April 17, 2023, and to
amend the insurance requirements of the original contract, to provide architectural
services for the CCTV- Generator Project, 10 Douglas Drive, Martinez. (100%
Public Education and Government Cable Access Funds)
C. 55 APPROVE and AUTHORIZE the Purchasing Agent, or designee, to execute, on
behalf of the Sheriff-Coroner, a purchase order amendment with Buchanan Food
Service to increase the payment limit by $125,000 to a new payment limit of
$500,000 and to extend the effective date through May 31, 2022, to provide bread
loaves, rolls and all related bakery items as needed for the West County, Martinez
and Marsh Creek detention facilities. (100% General Fund)
C. 56 APPROVE and AUTHORIZE the Purchasing Agent to execute, on behalf of the
Sheriff-Coroner, a purchase order amendment with Producers Dairy Products
Inc., to increase the payment limit by $70,000 to a new payment limit of $620,000
and to extend the effective date through May 31, 2022, for the purchase of dairy
products as needed for the County detention facilities. (100% General Fund)

execute a contract amendment with Valerie Gulyash , to increase the payment
limit by $150,000 to a new payment limit of $300,000 with no change in the
original term of January 1, 2021 through December 31, 2022, to provide
additional consulting and technical assistance for the Department’s Chargemaster
billing and EPIC Electronic Medical Record systems used at Contra Costa
Regional Medical Center and Contra Costa Health Centers. (100% Hospital

C. 58 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a novation contract with Paradise Adolescent Homes, Inc., in an amount
not to exceed $220,000 to provide a short term residential therapeutic program for
seriously emotionally disturbed youth for the period July 1, 2021 through June 30,
2022, including a six-month automatic extension through December 31, 2022 in
an amount not to exceed $110,000. (50% Federal Financial Participation, 50%
Mental Health Realignment)
C. 59 APPROVE and AUTHORIZE the Health Services Director, or designee, to
execute a contract with La Clinica De La Raza, Inc., in an amount not to exceed
$552,412 to provide primary care medical services for the Contra Costa CARES
