Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract #22–970–6 with Center for Human Development, a non-profit
corporation, in an amount not to exceed $329,638, to provide health education and outreach
services to residents of Contra Costa County, for the period from July 1, 2011 through June
30, 2012.
FISCAL IMPACT:
This Contract is funded by 29% Medi-Cal Administration Activities, 52% Health Services
Ambulatory Care, 7% by Health Services Community Wellness Prevention Nutrition
Program and 12% Kaiser Foundation Grant.
BACKGROUND:
On July 27, 2010, the Board of Supervisors approved Contract #22–970–4 (as amended by
Amendment Agreement #22-970-5) with Center for Human Development for the period
from July 1, 2010 through June 30, 2011, for the provision of health education and outreach
services to Spanish speaking residents of Contra Costa County.

Approval of Contract #22-970-5 will allow Contractor to continue providing services
through June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, the County will not be able to provide health education and
outreach services to Spanish speaking residents of Contra Costa County.
CHILDREN'S IMPACT STATEMENT:
This program supports the following Board of Supervisors’ community outcomes:
“Communities that are Safe and Provide a High Quality of Life for Children and Families”.
Expected program outcomes include an increase in nutritious food and physician activity.
