Approve and authorize the Health Services Director, or his designee, to submit, on behalf of
the County, a funding application #28-341 to the Federal Center for Disease Control (CDC)
for fiscal years 2011-2016, a 5 year Community Transformation Implementation Grant, in
an amount not to exceed $1.5 million per year, for a total grant application not to exceed
$7.5 million and to request the Chair of the Board to provide a letter of support for the
application.
FISCAL IMPACT:
Approval of submission of this application with result in an amount not to exceed $1.5
million per year, for a total grant application not to exceed $7.5 million from the Federal
Center for Disease Control (CDC). No County funds required.
BACKGROUND:
If awarded the CDC Transformation funding application will enable the Health Services
Department to implement a broad, evidence-based and practiced-based policy, and
programmatic, infrastructure changes to reduce risk factors responsible the leading causes
of death and

BACKGROUND: (CONT'D)
disability. The focus will be Tobacco-free living, Active living & healthful eating, and
high impact quality clinical preventive services, specifically, the prevention and control
of high blood pressure and high cholesterol. The 5 year funding period would begin no
later than 9/30/2011. No County match required.
CONSEQUENCE OF NEGATIVE ACTION:
If the request for approval to submit the grant application is not approved the Health
Department will not be eligible to apply, and potentially receive, a 5 year grant from the
CDC.
