Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Amendment Agreement #26-398-14 (V261S-0364) with the U.S.
Department of Veterans Affairs Northern California Health Care System (VANCHCS),
effective March 31, 2011, to amend Agreement #26-398-10 (as amended by Amendment
Agreements #26-398-11 through #26-398-13), to increase the total payment limit by
$80,000 from $168,000 to a new total of $248,000, and to extend the term of the agreement
to cover option year three through March 31, 2012.
FISCAL IMPACT:
The Amendment is funded in the Health Services Department's Enterprise I Budget and will
provide funding for option period year three from April 1, 2011 through March 31, 2012.
Under the term of this agreement, VANCHCS is paid a fee for service in accordance with
the fee schedule which is incorporated in the Contract. The services provided for the
County's patients under this Contract are billable to patients and third party payors.

For many years, the County and U.S. Department of Veterans Affairs have

BACKGROUND: (CONT'D)
maintained a mutual sharing agreement, which has made available to the County specialized
medical services not otherwise available due to lack of resources, equipment, and personnel.
These services included specialized laboratory testing, radiology services, nuclear medicine
studies, CT scans, MRI's, dermatology, gastroenterology, urology, audiology and speech,
and ophthalmology services.
On November 18, 2008, the Board of Supervisors approved Sharing Agreement #26-398-10
(as amended by Amendment Agreements #26-398-11 through #26-398-13), with
VACNHCS to provide specialized Dermatology services to County's patients referred for
treatment by Contra Costa Regional Medical Center physicians through March 31, 2011.
Approval of Amendment Agreement #26-398-14, will allow Contractor to continue to
provide specialized Dermatology services to County's patients referred for treatment by
Contra Costa Regional Medical Center physicians through March 31, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, County referred patients will not receive specialized
Dermatology services at VANCHCS.
