Cancellation Agreement #26-924-12 with Theresa Kailikole, DPM, a self-employed individual, effective on the close of business on May 31, 2011; and 2. Contract #26-924-13 with Theresa Kailikole, DPM, a self-employed individual, in an amount not to exceed $290,000, for the provision of professional Podiatry services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from June 1, 2011 through May 31, 2013. FISCAL IMPACT: 100% Enterprise Fund I. Costs depend upon utilization. As appropriate, patients and/or third-party payors will be billed for services. Contractor will be paid a per clinic rate instead of a monthly stipend. ATTESTED: June 21, 2011 David Twa, County Administrator and Clerk of the Board of Supervisors By: June McHuen, Deputy cc: Tasha Scott, Barbara Borbon C. 77 To:Board of Supervisors From:William Walker, M.D., Health Services Director Date:June 21, 2011 Contra Costa County Subject:Cancellation Agreement #26-924-12 and Contract #26-924-13 with Theresa Kailikole, DPM BACKGROUND: On January 12, 2010, the Board of Supervisors approved Contract #26-924-11 with Theresa Kailikole, DPM, to provide Podiatry services including consultation, clinic coverage and surgical procedures, for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from January 1, 2010 through December 31, 2011. In consideration of Contractor's agreement to continue providing services, both parties have agreed to renegotiate payment terms. This will allow the Contractor to be paid at a per clinic rate instead of a monthly stipend. The Department and the Contractor have mutually agreed to renegotiate the Contract Payment Provisions and the term to reflect the intent of the parties. Therefore, in accordance with Special Conditions, Paragraph 2. (Modification to Termination and Cancellation) of the Contract, the Department and Contractor have agreed to a mutual cancellation of Contract #26-924-11. Approval of Cancellation Agreement #26-924-12 will accomplish this termination. Under Contract #26-924-13, Contractor will provide professional Podiatry services, including consultation, clinic coverage and surgical procedures, through May 31, 2013. CONSEQUENCE OF NEGATIVE ACTION: If this contract is not approved, the Contractor would continue to be paid a monthly stipend for podiatry services at Contra Costa Regional Medical Center and Contra Costa Health Centers. CHILDREN'S IMPACT STATEMENT: Not applicable.
