Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #22–940–7 with Public Health Foundation
Enterprises, Inc. (dba PHFE Management Solutions), a non-profit corporation, effective
June 1, 2011, to amend Novation Contract #22–940–6, to increase the payment limit by
$8,715, from $150,097 to a new payment limit of $158,812, with no change in the original
term of July 1, 2010 through June 30, 2011, and to increase the automatic extension
payment limit by $2,179, from $37,524 to a new payment limit of $39,703, with no change
in the term of the automatic extension, through September 30, 2011.
FISCAL IMPACT:
This Contract is 100% federally funded, through the State, under Title III C 1 and Title III C
2 of the Federal Older Americans Act of 1965. No County funds are required. (No rate
increase)

BACKGROUND: (CONT'D)
#22–940–6 with Public Health Foundation Enterprises, Inc. (dba PHFE Management
Solutions), for the period from July 1, 2010 through June 30, 2011, which included a
three-month automatic extension through September 30, 2011, for coordination and
supervision of Meals on Wheels activities.
Approval of Contract Amendment Agreement #22–940–7 will allow the Contractor to
provide additional hours of services through June 30, 2011.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, County’s senior citizens who rely on the Senior
Nutrition Program for meals will not receive those meals.
