HomeMy WebLinkAboutMINUTES - 04122011 - C.54RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD), to execute, on behalf of the County, Contract Amendment Agreement #22–970–5 with Center for Human Development, a non-profit corporation, effective February 1, 2011, to amend Contract #22–970–4, to increase the payment limit by $107,000, from $191,903 to a new payment limit of $298,903, with no change in the original term of July 1, 2010 through June 30, 2011. FISCAL IMPACT: This amendment is funded 63% County funds and 37% Kaiser Permanente Community Grant. BACKGROUND: On July 27, 2010, the Board of Supervisors approved Contract #22–970–4 with Center for Human Development for the period from July 1, 2010 through June 30, 2011, for the provision of health education and outreach services.
