Broschard III, Fire Chief RE: Update on Fire Station Construction Projects ______________________________________________________________________  Fire Station 86 – Bay Point Following the January contract award, the project team held a kick off meeting. After the final utility placements, the project is anticipated to begin in March. The groundbreaking ceremony is scheduled for April 9, 2021, at 2 p.m.  Fire Station 70 – San Pablo Construction is in the final phases with punch list items currently being identified and addressed. We will celebrate the opening with a small, invitation only, COVID appropriate, ribbon cutting event on April 13, 2021, at 3 p.m.  Fire Station 9 – Pacheco Progress with this site has slowed as we work through drainage challenges and potential FAA approvals for an alternate site. All options are being pursued. A more detailed update should available in the coming months. Broschard III, Fire Chief RE: Fire Chief’s Report ______________________________________________________________________  Antioch Drive-By Shooting Incident. A Con Fire firefighter and an AMR paramedic were both struck by gunfire at the scene of an EMS call on Saturday evening, February 20, in Antioch. The firefighter was struck in the foot and the medic in the leg. They were both transported to local hospitals in stable condition, and neither injury was life-threatening. Both first responders were released from the hospital on February 21 and were recuperating at home as of the end of last month. The incident was an EMS call, originally reported to dispatch at 8:53 p.m., for a male patient with abdominal pain. Shots were reported by the Con Fire unit, at 9:08 p.m., shortly after firefighters and medics arrived on scene. In spite of the very fortunate outcomes in both cases, this was a very serious incident that we are not ignoring. W e are conducting an After Action Review to address the incident, including a review of existing policies and protocols to identify any changes that may be warranted. The incident was initially investigated by Antioch PD, with the Contra Costa County Sheriff taking the lead investigative role on February 21. The Sheriff announced a suspect had been arrested on Sunday morning in Richmond. The suspect was taken into custody following a pursuit from the scene in Antioch. The suspect has been publicly identified as 26-year-old Darryon Williams of Stockton. After being interviewed by detectives, W illiams was booked into the Martinez Detention Facility on one count of murder and eleven counts of attempted murder. At the end of February, he was being held in lieu of bail of approximately $11 million.  Evacuation Planning. The County Fire Chiefs were awarded a $300,000 Urban Areas Security Initiative (UASI) grant that was funded late last year. The procurement process is wrapping up, and a firm was selected to provide a countywide evacuation planning and operational softwa re system. This was an item of interest in Grand Jury Report No. 2007 issued in 2020. Implementation is expected in July 2021. Fire Chief’s Report – March 9, 2021 Page 2  Fire Station 4 Update. The District continues to prepare for the reopening of Fire Station 4 in unincorporated Walnut Creek as part of our overall G reat Recession recovery planning. Staffing of the station is planned for July 2022 which coincides with the elimination of debt service on our pension obligation bonds. Several logistical and facilities-related issues will need to be addressed over the next year as we prepare to reopen the station. It is anticipated overall engine response times will be reduced by an average of three to four minutes to the immediate are a once the station is reopened.  COVID-19 Vaccination Update. Efforts to vaccinate our employees continue as we begin to see benefits surrounding return-to-work protocols for fully vaccinated employees. As the fire Operational Area Coordinator for Contra Costa County, the District’s efforts to support County Health’s request for fire-hosted vaccination clinics continues. Clinics in east, west, and south county are being supported by firefighters, as well as those from all other fire agencies, the Office of the Sheriff, Brentwood PD, Los Medanos Community College Police, Hercules PD, CERT teams, County Medical Reserve Corps, and other agencies. Last week, we began to increase doses administered, from 500 per day to 1,000 per day, with a reduced number of total clinics and are prepared to expand the number of clinics as the situation warrants. The District is also engaged with County Health in planning for potential large scale vaccination sites. In February, the Fire District received approval from the State to operate a standalone vaccine clinic. We have procured an appropriate refrigeration unit, and all elements are in place to function as a separate and distinct clinic which would allow the District to order and receive vaccination directly from the supply chain. We do not currently plan to initiate this effort as the need to operate a standalone clinic is not necessary at this time, but, having received approval from the State, we have the option and flexibility to do so in the future.  Fire District Annexation Study Update. Phase II of this study continues with a rough draft and initial findings expected in April. Stakeholder interviews are taking place in March. As we progress, we anticipate additional stakeholder briefings and a number of public outreach activities for Con Fire and the other jurisdictions involved.  Operations Updates. Hoist training resumed for Con Air 1’s pilots in late February. The new estimate for completion of training is mid-April. An FAA issue has surfaced that may alter our initial hoist mission configuration. This issue requires administrative work to find a resolution but does not affect our ability to provide initial hoist training or impact our go-live date. A number of recent incidents, including a 10-rail-car incident at a local refinery, kept the HazMat crew busy in February. In the refinery incident, our crews made entry into the scene and secured a leaking valve. Fire Chief’s Report – March 9, 2021 Page 3 Our new HazMat apparatus is on site and being equipped. It should be inspected and inventoried by the state for OES typing in April after which it will go in service as our primary response unit. The OES apparatus we currently use for responses will be moved to reserve status for distant county OES responses or as an in-county back up. Our HazMat team participated in the BAY EX 2021 training that took place in Antioch and Vallejo last month. They worked alongside specialized units from the FBI and National Guard. The training focused on threats from chemical, biological, nuclear, radiological, and explosives. Follow-up training will occur in April at MOTCO in Concord. The recruitment and on-boarding process began last month for the 2021 Fire Control Worker program to staff our hand crew. A total of 84 interested candidates were interviewed for up to 24 positions. The estimated start date for this year’s program is May 1.  Alliance Ambulance Staffing. Alliance EMTs have been assigned to the FEMA Oakland Coliseum site to assist with vaccination efforts there. The Commission on Accreditation of Ambulance Services (CAAS) conferred another three-year accreditation on our emergency ambulance system. Congratulations to our EMS Division and our Alliance partner!  Fire Prevention Updates. In February, the District formally launched the Firewise USA® program districtwide. This is a national program that helps neighbors work together with their local fire jurisdiction to protect their communities from wildfire danger. It is administered by the Community Risk Reduction Unit of the Fire Prevention Bureau. The Firewise USA® recognition program provides a collaborative framework to help neighbors in a geographic area get organized, find direction, and take action to increase the fire resistance of their homes and community; and to reduce wildfire risks at the local level. Across our District, numerous communities face heightened wildfire danger due to terrain, vegetation, and local evacuation route conditions. Firewise USA® is a perfect complement to our many safety initiatives and a powerful tool for better protecting at-risk communities. Two new fire inspectors started last month and are assigned to the Engineering Unit where they will be working on new construction plan intake and reviewing plans for approval. Three additional fire inspectors are expected to be hired this month to fill Code Enforcement vacancies.  Communications Update. Since the universal power supply (UPS) failure at the Contra Costa Regional Fire Communications Center (CCRFCC), on January 24, we Fire Chief’s Report – March 9, 2021 Page 4 have temporarily replaced one of the two older units. Two brand new units were ordered for future installation. The Deputy Fire Chief and the new Assistant Fire Chief of Communications are working with County staff to expedite the design and eventual contracting of a major remodel to the CCRFCC. It is anticipated that we will repurpose our former administrative offices on Geary Road to relocate and accommodate the CCRFCC. Information Systems staff are working on several projects including full implementation of Everbridge mass text notifications, CAD interfaces, and filling current staff vacancies.  Training and Safety Updates. In February, the Training and Safety Division was very busy providing multiple hosted courses including All Hazards Incident Command, Basic Air Operations, Safety Officer, Advanced Drone Operations, Crew/Engine Boss, and W ildland Urban Interface Operations training. Our training captains, mechanics, and engineer’s cadre provided all probationary firefighters from Academies 54 and 55 with a 24-hour Apparatus Driver/Operator Training Course. The District’s Health and W ellness program continues to grow and provide expanding services to Con Fire personnel. In addition to providing a location for conduct of firefighter physicals and training, the wellness facility in our former fire prevention offices in Pittsburg is equipped with a variety of physical fitness equipment and is staffed by a physical therapist one day per week. The physical therapy evaluation and treatment program is off to an impressive start and will provide another valuable source of support for the organization. Congratulations to our Academy 55 firefighter/paramedics on the successful completion of their 10-month probationary training!  Apparatus Update. W e took delivery of two Pierce Type I fire engines in January, and they were placed in service last month at Fire Stations No. 5 in Pleasant Hill and No. 10 in Concord. This delivery continues the ongoing effort at replacing older apparatus and maintaining compliance with NFPA standards for taking aging apparatus out of frontline service. One replaced fire engine is a 2002 model with 180,000 miles that the District will surplus; and the other is a 2007 model that will be retained as a spare unit. RECOMMENDATION(S): APPROVE and AUTHORIZE the Fire Chief, or designee, to execute Amendment No. 2 to the Consulting Services Agreement with RRM Design Group, effective March 9, 2021, to increase the payment limit by $60,000, to a new payment limit of $1,300,000, to update the project personnel and billing rates to provide architectural services for Fire Station No. 9 in Pacheco and Fire Station No. 86 in Bay Point. Broschard III, Chief, Contra Costa Fire Protection District Date:March 9, 2021 Contra Costa County Subject:REACH Contract Amendment BACKGROUND: In March 2019, the District entered into an agreement with REACH Air Medical Services, LLC (“REACH”) to work cooperatively to establish a fire and air ambulance program to provide emergency and inter-facility air medical transportation to the residents of Contra Costa County and adjacent areas. Operations began in September 2019, and much work has been done to increase staffing from one air ambulance to a two-air ambulance operation at Buchanan Field in Concord. Aerial firefighting capacity was introduced in October 2020, and hoist rescue operations will commence in spring 2021. As a result of the first full year of joint operations of this unique public-private partnership, many lessons were learned and subsequent changes were made as we entered the second year of providing service together. As a result, the original agreement between the parties needs to be amended to recognize the contractual and financial obligations that have resulted from necessary operational, training, and logistical changes. The amendment, as proposed, outlines specific operational, staffing, and training responsibilities of both parties for air ambulance, aerial firefighting, and rescue operations. The amendment includes language for the District to provide medical oversight and direction for the program through the District's Medical Director. REACH will compensate the District $193.50 per hour for services provided under this section. The amendment adds a new provision for additional training capacity and reimbursement from REACH to the District for personnel in the amount of $13,282 per month. The aerial firefighting helicopter was assigned in 2020 to Cal OES-funded pre-positioning incidents. In addition, Cal Fire may choose to hire the District's helicopter for active fire incidents or to provide coverage for the Santa Clara Unit due to their helicopters being unavailable or committed on large incidents. These assignments provide funding of personnel and equipment and this amendment provides a mechanism for the District to reimburse REACH based on actual funds received from Cal OES or directly from Cal Fire for state-funded missions. Additionally, the roles and responsibilities of REACH and the District in support of both Cal OES and Cal Fire is specified within the amended agreement. A provision for the District to pay REACH for certain non-reimbursable or non-revenue generating flight time is included in this agreement. Excluding flight time attributable to training, such non-reimbursable flight time would generally fall under a search mission where the helicopter was used to assist in locating a stranded person with no medical need, the use of the helicopter for aerial observation of an incident, or the use of the helicopter on local firefighting missions where there is no reimbursement. Up to 90 hours of related flight time on an annual basis is allowed before this provision would apply or costs would be incurred by the District. If the annual threshold was met, the District would pay REACH $1,750 per hour for each additional hour of aircraft flight time. It is not anticipated the use of the helicopters on such incidents will exceed 90 hours per year. CONSEQUENCE OF NEGATIVE ACTION: Certain necessary operational, logistical, and staffing changes will not be able to be implemented to the District’s helicopter program. RECOMMENDATION(S): APPROVE and AUTHORIZE the Fire Chief, or designee, to apply for and accept grant funding from the U.S. Department of Homeland Security, Federal Emergency Management Agency, Assistance to Firefighters Grants Program in an amount not to exceed $867,000 for the purchase of a heavy rescue wrecker. Relevant incidents range from motor vehicle collisions requiring disentanglement to responses in industrial and refining facilities located in the County. A heavy rescue wrecker with a crane could have been used on the 2019 Nu Star petroleum tank farm explosions. This multi day incident and firefight included the explosion and fire of two 8.7 million gallon tanks. Once the fire was brought under control, the Contra Costa County Fire Protection District lacked the heavy equipment necessary to remove the heavy steel walls and lids in order to search for victims and conduct the corresponding fire investigation. The District had to wait for a privately hired contractor to bring in the cranes. According to the USGS the probability of at least one earthquake of magnitude 6.7 or larger in the San Francisco Bay Region is 72 percent, and the probability of at least one earthquake of magnitude 7.0 or larger is 51 percent. An earthquake of this magnitude has the potential to cause multiple building collapses in our jurisdiction. The heavy rescue wrecker can be used to save lives in this urban search and rescue scenario. RECOMMENDATION(S): APPROVE and AUTHORIZE the Auditor-Controller, on behalf of the Contra Costa County Fire Protection District, to make a payment to the State of California Department of Health Care Services in the amount not to exceed $572,744 for the Ground Emergency Medical Transportation Quality Assurance Fee for ambulance transports provided by the Contra Costa County Fire Protection District in the months of October through December (Quarter 4) of 2020. FISCAL IMPACT: The Ground Emergency Medical Transportation (GEMT) / Quality Assurance Fee (QAF) program increased the historic base rate of Medi-Cal reimbursement by $220.80 per transport. The GEMT/QAF program, therefore, increased the per transport reimbursement from the historic base rate of $118.20 to $339.00. To fund this program, the State of California Department of Health Care Services (DHCS) began assessing all transport providers within the State of California a fee of $25.23 per transport effective July 1, 2018. The fee was increased to $26.07 effective July 1, 2019 through June 30, 2020, and the fee was increased again to $32.30 effective July 1, 2020 through June 30, 2021. The increased fee of $32.30 was approved by the Centers for Medicare & Medicaid Services (CMS) on October 15, 2020. The fiscal impact will be revenue generating for the District due to the significant number of Medi-Cal transports within the District’s ambulance operating area. Medi-Cal is the second highest payer type (second to Medicare). The net revenue the District receives is calculated by multiplying the $220.80 per transport uplift for Medi-Cal fee-for-service and Medi-Cal managed care transports less the $32.30 QAF the District is required to pay on all transports. The net revenue estimate (uplift less QAF) for the District for 2020 is approximately $2 million under the current funding and fee parameters. BACKGROUND: CMS approved State Plan Amendment (SPA) 18-004 authorizing the GEMT/QAF program in March 2019. The Quality Assurance Fee (QAF) is an add-on fee used to increase Medi-Cal funding. Essentially, emergency medical transportation providers in California must pay this fee on all transports, and in return, providers will receive an uplift that increases the Medi-Cal reimbursement base rate for Medi-Cal fee-for-service and managed care recipients. CMS approved SPA 19-0020 in September 2019. SPA 19-0200 increased the QAF from $25.23 per transport to $26.07 per transport for ground emergency medical transports with dates of service beginning July 1, 2019 through June 30, 2020. SPA 20-0009 increased the QAF to $32.30 per transport for ground emergency medical transports with dates of service beginning July 1, 2020 through June 30, 2021. This request is for calendar year 2020 quarter 4 fees (for October-December 2020 transports). The payment is due on or before April 1, 2021. State program representatives indicated there would be no exceptions and late fees may be assessed by the State for payments not received by the due date. CONSEQUENCE OF NEGATIVE ACTION: The QAF payment is mandatory for all providers (public and private) within the State. If not paid, the State can assess late payment penalties and withhold provider payments for ambulance services provided to Medi-Cal recipients. The Notice of Intention to convey Offers of Dedications was duly published in the Contra Costa Times in compliance with Govt. Code Section 6061. On October 13, 2020, this Board approved the design and the bid documents for the construction of the Project; and on January 19, 2021, this Board awarded the contract to Overaa & Co. CONSEQUENCE OF NEGATIVE ACTION: If the Offers of Dedications are not approved, the Project will not meet the Conditions of Approval of Land Use Permit LP19-2014 and cause a delay in construction of the Project.
