Approve and authorize the Health Services Director or his designee, to execute, on behalf of
the County, Contract Amendment Agreement #76-515-4 with Larkin Breed, M.D., a
self-employed individual, effective December 1, 2010, to amend Contract #76-515-2, to
increase the payment limit by $1,500, from $375,000 to a new payment limit of $376,500,
with no change in the original term of January 1, 2008 through December 31, 2010.
FISCAL IMPACT:
100% Enterprise Fund I. Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services. No rate increase.
BACKGROUND:
On January 8, 2008, the Board of Supervisors approved Contract #76-515-2 with Larkin
Breed, M.D., for the period from January 1, 2008 through December 31, 2010, for the
provision of radiology services including consultation, on-call coverage and interpretation of
CT Scans, MRIs, Ultrasounds, invasive procedures and plain films at Contra Costa Regional
Medical Center and

BACKGROUND: (CONT'D)
Contra Costa Health Centers.
The County requested and the Contractor agreed to provide additional radiology coverage
during the Holidays which unfortunately exceeded his payment limit. Approval of Contract
Amendment Agreement #76-515-4 will allow the Contractor to provide additional radiology
services, through December 31, 2010.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, the Contractor would not be paid for radiology services
rendered at Contra Costa Regional Medical and Contra Costa Health Centers would go
untreated.
