Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Cancellation Agreement #26-535-4 with Nazia Choudhury, M.D., a
self-employed individual, effective at the close of business on January 1, 2011.
FISCAL IMPACT:
This Contract was funded 100% by Enterprise Fund I.
BACKGROUND:
On June 24, 2008, the Board of Supervisors approved Contract #26-535-1 (as amended by
Amendment Agreement #26-535-2 and #26-535-3) with Nazia Choudhury, M.D. for the
period from July 1, 2008 through June 30, 2011, for the provision of anesthesiology
services including consultation, training, medical procedures, on-call and clinic coverage at
Contra Costa Regional Medical Center and Contra Costa Health Centers.
In accordance with General Conditions Paragraph 5 (Termination and Cancellation), of the
Contract, the Department and Contractor have agreed to a mutual cancellation of this
Contract. Approval

CONSEQUENCE OF NEGATIVE ACTION:
If this termination is not approved, then Department cannot reflect the change in the annual
budget and hire a replacement anesthesiologist.
