Approve and authorize the Health Services Director, or his designee (Patricia Tanquary) to execute on behalf of the
County, Contract #27-659-3 with Juan Anaya, OD (dba San Pablo Optometry Center), a sole proprietor, in an amount
not to exceed $130,000, to provide professional optometry services for the Contra Costa Health Plan for the period
from January 1, 2011 through December 31, 2012.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon
utilization. (No rate increase)
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care services for its members
under the terms of their Individual and Group Health Plan membership contracts with the County. On January 20,
2009, the Board of Supervisors approved Contract #27-659-2 with Juan Anaya, OD (dba San Pablo Optometry
Center), for the period from January 1, 2009 through December 31, 2010, to provide optometry services. Approval of
Contract #27-659-3 will allow the Contractor to continue to provide optometry services through December 31, 2012.
