Approve and authorize the Health Services Director or his designee, to execute, on behalf of
the County, Contract Amendment Agreement #26-817-33 with East Bay Nephrology
Medical Group, Inc., to amend Contract #26-817-32, effective October 1, 2010, to increase
the total Payment Limit by $18,900 from $122,400 to a new total payment limit of $141,300
with no change in the original term of the Contract from July 1, 2009 through June 30,
2012.
FISCAL IMPACT:
This Contract is funded by 100% Enterprise Fund I. Cost to the County depends upon
utilization. As appropriate, patients and/or third party payors will be billed for services. No
increase in rates.
BACKGROUND:
On June 16, 2009, the Board of Supervisors approved Contract #26-817-32 with East Bay
Nephrology Medical Group, Inc., to provide professional Nephrology services, including
consultation, training and/or medical procedures for patients at Contra Costa Regional
Medical Center and Contra Costa Health Centers, for the period from July 1, 2009 through

Approval of Contract Amendment Agreement #26-817-33 will amend the contract and
allow the Contractor to provide additional nephrology services at Contra Costa Regional
Medical Center and Contra Costa Health Centers, through June 30, 2012.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients requiring these medical services at Contra Costa
Regional Medical and Contra Costa Health Centers would go untreated.
