HomeMy WebLinkAboutMINUTES - 11092010 - C.43RECOMMENDATION(S): Acknowledge receipt of notice from Kamyar Farhangfar, M.D., Inc. (dba Rapidcare Medical Clinic), a professional corporation, requesting termination of Contract #27-522-6, for the provision of professional primary care and urgent care services, effective at the close of business on November 1, 2010. FISCAL IMPACT: This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon utilization. BACKGROUND: On December 16, 2008, the Board of Supervisors approved Contract #27-522-6 with Kamyar Farhangfar, M.D., Inc. (dba Rapidcare Medical Clinic) for the period from November 1, 2008 through October 31, 2010, to provide professional primary care and urgent care services for Contra Costa Health Plan members. The purpose of this Board Order is to advise the Board of Supervisors that the Department and the Contractor have agreed to terminate Contract #27-522-6, effective at the close of business on November 1, 2010.
