Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Standard Agreement #28 816 with the State Department of
Health Care Services, to pay the County an amount not to exceed $42,000, for the provision
of a pilot Collaborative Primary Care Integration (CPCI) and Mental Health Services
Project, for the period from June 15, 2010 through December 31, 2011.
FISCAL IMPACT:
Approval of this Agreement will result in a total payment to the County from the State
Department of Health Care Services, in an amount not to exceed $42,000 for the CPCI and
Mental Health Services Project through December 31, 2011. No County match is required.
BACKGROUND:
Contra Costa County Mental Health Services and State Primary Care Health are partnering
to integrate mental and physical health care service. This collaborative project is needed to
improve the identification and treatment of cardiovascular disease, diabetes, and related

BACKGROUND: (CONT'D)
risk factors in the Medi-Cal or public assistance population with serious mental illness
(SMI). Project results will improve understanding of how to achieve clinical improvement
for a highly costly and at-risk Medi-Cal and/or public assistance population, for whom at
least two major health care systems share service responsibilities. In addition, project
partners will test and implement improvement practices in their organizations to achieve
better health outcomes, enhanced integration of primary care and mental health services and
lasting change at the practice level. Although the improvements should apply to many
aspects of care, the initial focus will be on improving, by at least 50%, the identification and
treatment of cardiovascular disease and its risk factors, including physical inactivity,
smoking, obesity, diabetes, hypertension, and dyslipidemia in the population of individuals
with SMI identified at pilot sites.
The goal of this Project is to improve the health of individuals with severe mental illness
and co-occurring chronic medical disorders through more effective partnerships between
mental health and primary care providers. Approval of Standard Agreement #28-816 will
allow County to receive funding for Collaborative Primary Care Integration (CPCI) and
Mental Health Services Project, including agreeing to indemnify and hold the State harmless
for claims arising out of the County’s performance under the Contract through December
15, 2011.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, individuals with severe mental illness and co-occurring
chronic medical disorders will not receive the necessary assistance to help improve their
health through a more effective partnerships between mental health and primary care
providers.
