Approve and authorize the Health Services Director, or his designee (Patricia Tanquary) to
execute on behalf of the County, Contract #27–174–6 with Edward Connolly, M.D., a
self-employed individual, in an amount not to exceed $150,000, to provide professional
pediatric primary care services for the Contra Costa Health Plan for the period from
October 1, 2010 through September 30, 2012.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member
premiums. Costs depend upon utilization. No change in rates.
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care
services for its members under the terms of their Individual and Group Health Plan
membership contracts with the County. On October 28, 2008, the Board of Supervisors
approved Contract #27–174–5 with Edward Connolly, M.D. for the period from October 1,
2008 through September 30, 2010, to provide professional pediatric primary care services.
Approval of Contract #27–174–6 will allow the Contractor to continue providing services
