Approve and authorize the Health Services Director, or his designee (Wendel Brunner,
M.D.), to execute on behalf of the County, Amendment Agreement #28-700-12 (State #EPO
P4-08) with the California Department of Public Health, effective July 30, 2010, to amend
Agreement #28-700-11 with no change in the original amount of $3,680,915 payable to
County, for the Public Health Emergency Preparedness Comprehensive Program, Public
Health Emergency Response (PHER) H1N1, Phases one (1) through four (4) to extend the
term from July 30, 2010 through July 30, 2011.
FISCAL IMPACT:
This is a no cost extension. Approval of this Agreement will result in no change in the
original funding amount not to exceed $3,680,915 payable to County from the California
Department of Public Health for the Public Health Emergency Response (PHER) H1N1. No
County funds are required.

Subject:Amendment Agreement #28-700-12 with the California Department of Public Health
BACKGROUND: (CONT'D)
with the California Department of Public Health to fund multiple Public Health Emergency
Preparedness activities including, but not limited to, the Centers for Disease Control (CD)
preparedness activities, the Cities Readiness Initiative (CRI) activities, Pandemic Influenza
preparedness and ASPR Hospital Preparedness Program activities, for the County’s Public
Health Emergency Preparedness Response Program, Phase one (1) through Phase four (4)
through July 30, 2010.
The County Public Health Division was recently notified by the California Department of
Public Health that an extension of time was granted for continuation of the Public Health
Emergency Response H1N1 Program. Approval of this Amendment Agreement #28-700-12
will allow continuous funding for services through July 30, 2011.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, the Division will not be able to continue providing the
necessary services to County residents and the funds must be spent by July 30, 2011.
