Approve and authorize the Health Services Director or his designee, to execute, on behalf of
the County as follows:
1. Cancellation Agreement #26 968-11 with Thomas Paige, M.D., a self-employed
individual, effective on the close of business on September 30, 2010; and
2. Contract #26-968-12 with Thomas Paige, M.D., a self-employed individual, in an amount
not to exceed $348,000, for the provision of professional dermatology services for patients
at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period
from October 1, 2010 through September 30, 2013.
FISCAL IMPACT:
This Contract is funded by Enterprise I Budget. Cost to the County depends upon
utilization. As appropriate, patients and/or third party payors will be billed for services. Rate
is changing from a monthly rate to a per clinic rate and includes additional administrative
duties. No increase in cost to County.

Subject:Cancellation Agreement #26-968-11 and Contract #26-968-12 with Thomas Paige, M.D.
BACKGROUND:
On May 12, 2009, the Board of Supervisors approved Contract #26-968-10 with Thomas
Paige, M.D., to provide dermatology services including consultation, training, on-call,
medical and/or surgical procedures, and clinic coverage, for patients at Contra Costa
Regional Medical Center and Contra Costa Health Centers, for the period from June 1, 2009
through May 31, 2012.
In consideration of Contractor’s agreement to continue providing services, both parties have
agreed to re-negotiate payment terms. This will allow the Contractor continue to be paid at a
per clinic rate and provide additional administrative duties. Therefore, in accordance with
Special Conditions, Paragraph 2. (Modification to Termination and Cancellation) of the
Contract, the Department and Contractor have agreed to a mutual cancellation of Contract
#26-968-10. Approval of Cancellation Agreement #26-968-11 will accomplish this
termination.
Under Contract #26-968-12, Contractor will provide professional dermatology services,
including, consultation, clinic coverage, training, on-call services, and medical and/or
surgical procedures, through September 30, 2013.
CONSEQUENCE OF NEGATIVE ACTION:
If this contract is not approved, patients requiring these medical services at Contra Costa
Regional Medical and Contra Costa Health Centers would go untreated.
