Approve and authorize the Health Services Director, or his designee (Donna Wigand), to
execute, on behalf of the County, Contract Amendment Agreement #74–317-3 with
Alternative Family Services, Inc., a non-profit corporation, effective July 1, 2009, to amend
Novation Contract #74–315–2, to add additional case management service with no change
in the payment limit of $500,000, no change in the original term of July 1, 2009 through
June 30, 2010, and no increase in the automatic extension payment limit of $250,000,
through December 31, 2010.
FISCAL IMPACT:
This Contract is funded 49% by Federal FFP Medi-Cal, 26% by State Early and Periodic
Screening, Diagnosis, and Treatment (EPSDT), 20% by Individuals with Disabilities
Education Act (IDEA/SB 90), and 5% by Mental Health Realignment. No increase in rates.

BACKGROUND: (CONT'D)
#74–315–2 with Alternative Family Services, Incorporated for the period from July 1, 2009
through June 30, 2010, which included a six-month automatic extension through December
31, 2010, for the provision of a day treatment program for SED adolescents.
Approval of Contract Amendment Agreement #74–317-3 will allow the Contractor to
provide additional services to Multidimensional Treatment Foster Care services to SED
youth, through June 30, 2010.
CONSEQUENCE OF NEGATIVE ACTION:
If this amendment is not approved, contractor will not be able to provide additional case
management services to seriously emotionally distrubed children.
