Approve and authorize the Health Services Director or his designee, to execute, on behalf of
the County, Contract #26-672 with Siri Sunderi Cheng, M.D., Inc., a self-employed
individual, in an amount not to exceed $936,000 to provide professional otolaryngology
services at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the
period from October 1, 2010 through September 30, 2013.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I. Cost to the County depends upon
utilization. As appropriate, patients and/or third party payors will be billed for services.
BACKGROUND:
For a number of years the County has contracted with Medical, Dental and Mental Health
Specialists to provide specialized professional services that are not otherwise available in its
Hospital and Health Centers.
Under Contract #26-672, Dr. Cheng will provide professional otolaryngology services
including
