Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #72-003-1 with Contra Costa Association
for Retarded Citizens, Inc., a non-profit corporation, effective June 1, 2010, to amend
Contract #72-003, to increase the payment limit by $20,000, from $73,300 to a new
payment limit of $113,300 and extend the term from July 30, 2010 through September 30,
2010.
FISCAL IMPACT:
This Contract is funded 100% by State Department of Public Health.
BACKGROUND:
In July 2010, the County Administrator approved and the Purchasing Services Manager
executed Contract #72-003 with Contra Costa Association for Retarded Citizens, for the
period from March 1, 2010 through July 30, 2010, for the provision of H1N1 vaccination
outreach and education services to Contra Costa County residents including providing
information on exposure, vulnerability, prevention methods and locations of free
vaccinations.

BACKGROUND: (CONT'D)
Approval of Contract Amendment Agreement #72-003-1 will allow the Contractor to
provide additional services including conducting a survey and providing a written report
with regard to availability of developmental disabled residents on how to access H1N1
information from the County’s website, through September 30, 2010.
CONSEQUENCE OF NEGATIVE ACTION:
If the recommendation is not approved the survey will not be completed and vital
information will not be received.
