Approve and authorize the Health Services Director or his designee (Wendel Brunner, MD),
to execute, on behalf of the County, Interagency Agreement #28–756–4 with First 5 Contra
Costa Children and Families Commission, to pay the County an amount not to exceed
$344,793 for the Medically Vulnerable Infant Program, Home Visiting Services Project, for
the period from July 1, 2010 through June 30, 2011.
FISCAL IMPACT:
Approval of this Agreement will result in $344,793 from Contra Costa Children and
Families Commission for the Medically Vulnerable Infant Program – Home Visiting
Services Project. No County funds are required.
BACKGROUND:
Contra Costa Children & Families Commission, in collaboration with the County's Family,
Maternal and Child Health Programs will coordinate and administer the Medically
Vulnerable Infant Program – Home Visiting Services Project to support medically
vulnerable infants, defined as newborns that are at risk for neuron-developmental delays as
a result of medical

On September 15, 2009, the Board of Supervisors approved Agreement #28–756–3 with
First 5 Contra Costa Children and Families Commission.
Approval of Interagency Agreement #28–756–4 will provide continuous coordination and
support of local programs and services designed to help children 0-5 reach their potential in
school and life through the development of a seamless, comprehensive system of
information and services through June 30, 2011.
