Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #26-875-21 with Paul H. Kwok, D.O., a
self-employed individual, effective August 1, 2010, to amend Contract #26-875-18 (as
amended by Amendment Agreement #26-875-20) to increase the payment limit by $14,000,
from $1,390,000 to a new payment limit of $1,404,000, with no change in the original term
of October 1, 2007 through September 30, 2010.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I. No change in rates. Increase is due to
additional operating room coverage services at Contra Costa Regional Medical Center.
BACKGROUND:
On December 18, 2007, the Board of Supervisors approved Contract #26-875-18 (as
amended by Amendment Agreement #26-875-20) with Paul H. Kwok, D.O., for the period
from October 1, 2007 through September 30, 2010, for the provision of anesthesiology
services including consultation,

At the time of negotiations the Payment Limit was based on target levels of utilization.
However, the utilization of operating room coverage was higher than originally anticipated.
Approval of Contract Amendment Agreement #26-875-21 will allow the Contractor to

Contra Costa Health Centers, through September 30, 2010.
