Approve and authorize the Health Services Director, or his designee (Patricia Tanquary) to
execute on behalf of the County, Contract #27–480–7 with K.H. Connie Wang-Chen, O.D.,
a sole proprietor, in an amount not to exceed $120,000, to provide professional Optometry
services for the Contra Costa Health Plan for the period from August 1, 2010 through July
31, 2012.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member
premiums. Costs depend upon utilization.
BACKGROUND:
The Health Plan has an obligation to provide certain specialized professional health care
services for its members under the terms of their Individual and Group Health Plan
membership contracts with the County. On September 16, 2008, the Board of Supervisors
approved Contract #27–480–6 with K.H. Connie Wang-Chen, O.D., to provide optometry
services for Contra Costa Health Plan members, for the period from August 1, 2008 through
July 31, 2010, to provide
