Approve and authorize the Health Services Director, or his designee (Pat Godley), to
execute, on behalf of the County, Contract Amendment Agreement #23–454–2 with Terra
Verde, LLC, a limited liability company, effective September 16, 2009, to amend Contract
#23–454 (as amended by Contract Amendment Agreement #23–454–1), to increase the
payment limit by $50,000, from $100,000 to a new payment limit of $150,000 and extend
the trerm from May 31, 2010 through May 31, 2011.
FISCAL IMPACT:
This Contract is funded 100% by budgeted Enterprise Fund I.
BACKGROUND:
In August 2009, the County Administrator approved, and the Purchasing Services Manager
executed Contract #23–454 (as amended by Contract Amendment Agreement #23–454–1)
with Terra Verde, LLC for the period from June 1, 2009 through May 31, 2010, for the
provision of consultation and technical assistance to the Department’s Health Services

Approval of Contract Amendment Agreement #23–454–2 will allow the Contractor to
