Approve and authorize the Health Services Director, or his designee (Wendel Brunner,
MD), to execute, on behalf of the County, Contract #22–970–4 with Center for Human
Development, a non-profit corporation, in an amount not to exceed $191,903, to provide
health education and outreach services for the period from July 1, 2010 through June 30,
2011.
FISCAL IMPACT:
This Contract is 45% Medi-Cal Administration Activities, 45% Kaiser Grant, and 10% by
Community Wellness Prevention Nutrition Grant. There is no increase in contract rates.
BACKGROUND:
This Contract meets the social needs of County’s population in that it provides culturally
sensitive, community-oriented health outreach and education services to residents of Contra
Costa County through the Promotora and African American Health Conductors Program,
targeting both the Spanish-speaking population and African American population, as a part
of the Department’s

BACKGROUND: (CONT'D)
Reducing Health Disparities Initiative.
On March 23, 2010, the Board of Supervisors approved Contract #22 970-2 (as amended by
Contract Amendment/Extension Agreement #22–970–3), with Center for Human
Development, for the period from July 1, 2009 through June 30, 2010, for the provision of
health education and outreach services.
Approval of Contract #22–970–4 will allow the Contractor to continue providing services
through June 30, 2011.
