Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #26-883-18 with Rodney J. Chan, M.D., a
self employed individual, effective May 1, 2010, to amend Contract #26-883-17 to increase
the payment limit by $5,000, from $288,000 to a new payment limit of $293,000, with no
change in the original term of June 1, 2009 through May 31, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I. Increase in Payment Limit is due to
additional clinic coverage and surgeries to accommodate increase in patient load. No change
in rate of pay.
BACKGROUND:
On May 12, 2009, the Board of Supervisors approved Contract #26-883-17 with Rodney J.
Chan, M.D., for the period from June 1, 2009 through May 31, 2011, for the provision of
podiatry services including consultation, clinic coverage, training, on-call services and
medical and/or surgical procedures.
