HomeMy WebLinkAboutMINUTES - 06222010 - C.82RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on behalf of the County, Contract Amendment Agreement #74-271-72(4) with Juanita Knight, MFT, a self-employed individual, effective June 1, 2010, to amend Contract #74-271-72(2) [as amended by Contract Amendment Agreement #74-271-72(3)], to increase the payment limit by $30,000, from $90,000 to a new payment limit of $120,000, with no change in the original term of July 1, 2009 through June 30, 2011. FISCAL IMPACT: This Contract is funded 100% by Medi-Cal Funds, offset 50% State and 50% Federal. BACKGROUND: In June 2009, the County Administrator approved and the Purchasing Services Manager executed Contract #74-271-72(2) [as amended by Contract Amendment Agreement #74-271-72(3)] with Juanita Knight, MFT, to provide Medi-Cal specialty mental health services, for the period from July 1, 2009 through June 30, 2011.
