Approve and authorize the Health Services Director, or his designee (Donna Wigand), to
execute, on behalf of the County, Contract Amendment Agreement #24–928–16 with Fred
Finch Children’s Home, Inc., a non-profit corporation, effective April 1, 2010, to amend
Novation Contract #24–928–15, to increase the payment limit by $138,812, from $415,693
to a new payment limit of $554,505, with no change in the original term of July 1, 2009
through June 30, 2010, and no change in the automatic extension through December 31,
2010 in the amount of $207,847.
FISCAL IMPACT:
This Contract is funded 56% by Federal Medi-Cal Revenue, 35% by State Early and
Periodic Screening, Diagnostics, and Treatment (EPSDT), 5% by Individuals with
Disabilities Education Act (IDEA/SB 90), and 4% by Mt. Diablo Unified School District.
No County funds are required.

BACKGROUND: (CONT'D)
#24–928–15with Fred Finch Children’s Home, Inc. for the period from July 1, 2009 through
June 30, 2010, which included a six-month automatic extension through December 31,
2010, for the provision of school-based day treatment and mental health services for
students in the Mt. Diablo Unified School District.
Approval of Contract Amendment Agreement #24–928–16 will allow the Contractor to
provide services to additional students through June 30, 2010.
