Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #26-904-20 with Stephen D. Weiss, M.D.,
a self-employed individual, effective April 1, 2010, to amend Contract #26-904-19, to
increase the payment limit by $60,000, from $748,000 to a new payment limit of $808,000,
with no change in the original term of June 1, 2009 through May 31, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I.
BACKGROUND:
On June 9, 2009, the Board of Supervisors approved Contract #26-904-19 with Stephen D.
Weiss, M.D. for the period from June 1, 2009 through May 31, 2011, for the provision of
general surgery services including consultation, training, on-call, medical and/or surgical
procedures, and clinic coverage, for patients at Contra Costa Regional Medical Center and
Contra Costa Health Centers.
