FISCAL IMPACT: The total annual cost for this request is approximately $448,765 for the Rehabilitation Therapy Services Program and $453,349 for Public Health's CCS Therapy Services Program, totaling $902,114. Annual cost may be offset by Registry Costs paid by the Rehabilitation Therapy Services Program (approximately $296,770), per diem salaries paid (approximately $703,696), and registry costs paid by Public Health during Fiscal Year 2008-2009 (approximately $240,000) totaling $1,000,466 for the period of August 1, 2008 through August 30, 2009. The cost associated with retroactively establishing the March 1, 2010 effective date is approximately $49,616. For a number of years, the Rehabilitation Therapy Services Program has been understaffed, and has experienced difficulties with recruitment and retention of qualified therapy personnel, primarily due to the current low salaries. According to a recent market survey, Contra Costa County, in comparison to the (9) Bay Area Counties is below market levels for similar Therapy Services classifications. Additionally, the department has been in constant competition with the surrounding local hospitals such as Kaiser and John Muir, who offer higher pay. Compensation is an important factor when addressing staff recruitment and retention. Over the past two years, 24 licensed therapists including two supervisors have resigned, which impacted the Therapy Assistant and Therapy Aide classifications. By regulations, Therapy Assistants and Aides cannot perform their duties without a licensed therapist in a clinic. The loss of therapy personnel resulted with the temporary closure of rehabilitation therapy services in the Pittsburg Health Center and disruption to patient care services in general. Consequently, to continue providing a certain level of patient care, the use of per diem employees has increased significantly over the years. Local One filed an Unfair Labor Practice charge against the county regarding the use of Per Diem employees. Both the County and Public Employees Union Local One has been discussing the matter in order to reach a mutual settlement. Public Health's California Children Services Therapy Program has numerous positions in the same classifications, which will receive the same salary reallocations. Additionally, the supervisory and managerial classifications will also receive increases to maintain internal alignments. With the exception of the supervisor and director classifications, the therapist series are included in the coalitions "shared sacrifice" exemption list. The Health Services Department is requesting the approval of this action, in order to bring Contra Costa rehabilitation therapy salaries to a competitive market level, enable the department to fill current vacancies, retain existing staff and reduce the use of per diem personnel.
