Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Contract Amendment Agreement #74-271-82(3) with Lori
Salinas, MFT, a self-employed individual, effective April 1, 2010, to amend Contract
#74-271-82 [as amended by Contract Amendment Agreements #74-271-82(1) and
74-271-82(2)], to increase the payment limit by $25,000, from $120,000 to a new payment
limit of $145,000, with no change in the original term of July 1, 2008 through June 30,
2010.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds, offset 50% State and 50% Federal.
BACKGROUND:
In June 2008, the County Administrator approved and the Purchasing Services Manager
executed Contract #74 271 82 [as amended by Contract Amendment Agreements
#74-271-82(1) and 74-271-82(2)] with Lori Salinas, MFT, for the period from July 1, 2008
through June 30, 2010, to provide Medi-Cal specialty mental health services. At the time of
