Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Contract Amendment Agreement #74-108-6(4) with
Hiawatha Harris, M.D., Inc. (dba Pathways to Wellness Medication Clinic), a corporation,
effective April 1, 2010, to amend Contract #74-108-6(3), to increase the payment limit by
$1,300,000, from $700,000 to a new payment limit of $2,000,000, with no change in the
original term of July 1, 2009 through June 30, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds, offset 50% State and 50% Federal.
BACKGROUND:
On August 11, 2009, the Board of Supervisors approved Contract #74-108-6(3) with
Hiawatha Harris, M.D., Inc. (dba Pathways to Wellness Medication Clinic), to provide
Medi-Cal specialty mental health services, for the period from July 1, 2009 through June
30, 2011. At the time of negotiations, the payment limit was based on target levels of
utilization. However, the utilization
