SUBJECT: Approval of Funding Proposal for Preventive Health Care for the Aging
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDATION
APPROVE submission of Funding Proposal to the State Department of Health Services,
as follows:

II. FINANCIAL IMPACT.
Funding: $60,000.00 State
$62,730.44 County In-Kind
$122,730.44 Total Program Cost
The proposal requests State funding for FY 85-86. If approved by the State, the
revenue and local match will be budgeted in the 1985-86 budget submitted by the
Health Services Department.
III. REASONS FOR RECOMMENDATION.
Based on 1980 census data, 13% of the population or 89,236 persons sixty years of
age or older reside in Contra Costa County. By the year 2000, seniors will consti-
tute nearly 20% of the County's total population. These changes can be expected to
have significant ramifications for the future provision of health and human services
to the elderly. A great proportion of seniors cannot afford "wellness care" and
early symptoms of health problems, including hypertension, glaucoma, adult onset
diabetes and nutritional deficiencies go unnoticed or ignored. This proposal, if
funded by the State, will increase the effectiveness of the department in providing
preventive health education and health promotion activities to the elderly.

IV. BACKGROUND.
Information and referral services by the Area Agency on Aging, the Nutrition Project
for the Elderly, the Meals-On-Wheels Program, and annual Health Fairs are among the
resources that provide services to the elderly. The Nutrition Project for the
Elderly provides meals to low-income seniors but not individual counseling. Public
Health Nurses relate on a limited basis with the majority of senior nutrition sites,
senior activity centers, and senior housing sites. The services provided and the
health promotion activities need to be increased at all sites in order to effect a
change in health related behavior of senior participants. The purpose of the
attached proposal is to request State funding to help senior citizens maintain or
improve their health through health problem identification, referral for problems
found, health counseling and education. The attached Sixteen Point Narrative
provides additional information on the proposed program.
The County's in-kind contribution to this program will include rent and utilities,
administrative costs, office supplies, and minor medical supplies such as tongue
depressors and stethoscopes. There will be no increase in the Public Health budget.
V. CONSEQUENCES OF NEGATIVE ACTION.
Failure to approve this funding proposal will limit the provision of preventive
health care for the elderly to current County program activities. Accomplishment of
the objectives set forth in the Sixteen Point Narrative will not be possible without
additional funding proposed under this project.
This document has been approved by the Department's Contracts and Grants Administrator in
accordance with the guidelines approved by the Board's Order of December 1, 1981
(Guidelines for contract preparation and processing, Health Services Department).
In_order to meet the State's deadline for submission, draft copies of the funding propo-
sal have already been forwarded to the State Department of Health Services, but subject
to Board approval. Four copies of the Board Order authorizing submission of the proposal
to the State should be returned to the Contracts and Grants Unit for transmittal to the
State. The attached copy of the Funding Proposal should be retained by the Clerk of the
Board for County files.
