Approve and authorize the Health Services Director, or his designee (Patricia Tanquary) to
execute on behalf of the County, Contract Amendment Agreement #27–611–4 with Norman
B. Livermore, III, MD, a self-employed individual, effective February 1, 2010, to amend
Contract #27-611-3, to increase the payment limit by $80,000, from $80,000 to a new
payment limit of $160,000, with no change in the original term of October 1, 2008 through
September 30, 2010.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member
premiums. Costs depend upon utilization.
BACKGROUND:
In March 2009, the County Administrator approved and the Purchasing Services Manager
executed Contract #27–611–3 with Norman B. Livermore, III, MD, for the period from
October 1, 2008 through September 30, 2010, to provide professional orthopedic services
for Contra Costa Health Plan members.At the time of negotiations, the payment limit was
based on target
