HomeMy WebLinkAboutMINUTES - 03092010 - C.40RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Patricia Tanquary) to execute on behalf of the County, Contract Amendment Agreement #27–611–4 with Norman B. Livermore, III, MD, a self-employed individual, effective February 1, 2010, to amend Contract #27-611-3, to increase the payment limit by $80,000, from $80,000 to a new payment limit of $160,000, with no change in the original term of October 1, 2008 through September 30, 2010. FISCAL IMPACT: This Contract is funded 100% by Contra Costa Health Plan (Health Plan) member premiums. Costs depend upon utilization. BACKGROUND: In March 2009, the County Administrator approved and the Purchasing Services Manager executed Contract #27–611–3 with Norman B.
