Approve and authorize the Health Services Director, or his designee (Donna Wigand), to
execute, on behalf of the County, Contract Amendment Agreement #74–315–4 with
Community Options for Families and Youth, Incorporated, a non-profit corporation,
effective February 1, 2010, to amend Novation Contract #74–315–3, to increase the
payment limit by $600,000, from $1,200,000 to a new payment limit of $1,800,000, with no
change in the original term of July 1, 2009 through June 30, 2010, and to increase the
automatic extension payment limit by $300,000, from $600,000 to a new payment limit of
$900,000, with no change in the term, through December 31, 2010.
FISCAL IMPACT:
This Contract is funded 62% by Federal FFP Medi-Cal, 33% by State Early and Persistent
Screening, Diagnosis, and Treatment (EPSDT), and 5% by Mental Health Realignment.

BACKGROUND: (CONT'D)
#74–315–3 with Community Options for Families and Youth, Incorporated for the period
from July 1, 2009 through June 30, 2010, which included a six-month automatic extension
through December 31, 2010, for the provision of Therapeutic Behavioral Services (TBS).
Approval of Contract Amendment Agreement #74–315–4 will allow the Contractor to
provide TBS to additional youth, through June 30, 2010.
