Approve and authorize the Health Services Director or his designee (Wendel Brunner,
M.D.), to accept on behalf of the County Grant Award #28-319, an amount not to exceed
$25,000 from Kaiser Permanente, Community Benefit Diablo Area, for the Promotora
Alma Con Alma Peer Health Support group Project, for the period from July 1, 2009
through June 30, 2010.
FISCAL IMPACT:
Approval of this Grant Award will result in an amount not to exceed $25,000 from Kaiser
Permanente, Community Benefit Diablo Area, for the period from July 1, 2009 through
June 30, 2010. (No County funds are required)
BACKGROUND:
The Health Services Department’s Pittsburg Health Center, Bay Point Health Center and
Public Health Division diabetes staff will work in collaboration to improve the health status
of forty (40) Hispanic community residents through impact of peer health support group and
navigation assistance, to combat high incidence of diabetes, obesity and other chronic
diseases

BACKGROUND: (CONT'D)
among Hispanic residents. The goal of the project will be to create a venue where East
Contra Costa Latinos who may or may not have insurance or have high literacy ability or
speak English, can improve health outcomes through the positive dynamics of a pear health
support group model.
The County provided services in good faith until grantor provided funding for the use of this
project.
Approval of Grant Award #28-319 will support this project and allow the Contra Costa
Health Services to create a safe and supportive, culturally and linguistically appropriate peer
health support model, with an emphasis on diabetes and nutrition, through June 30, 2010.
