Approve and authorize the Health Services Director, or his designee (Wendel Brunner,
MD), to execute, on behalf of the County, Contract Amendment Agreement #22–986–1
with Monument Community Partnership, a non-profit corporation, effective October 1,
2009, to amend Contract #22–986, to increase the payment limit by $67,500, from $118,001
to a new payment limit of $185,501, with no change in the original term of July 1, 2009
through June 30, 2010.
FISCAL IMPACT:
This Contract is funded 64% by California Nutrition Network and 36% by the Hospital
Council of Northern and Central California.
BACKGROUND:
On June 16, 2009, the Board of Supervisors approved Contract #22–986 with Monument
Community Partnership for the period from July 1, 2009 through June 30, 2010, for the
provision of consultation and technical assistance to the Department with regard to ensuring
County and its providers are meeting the requirements of the United States Department of
Agriculture and
