HomeMy WebLinkAboutMINUTES - 11102009 - C.43RECOMMENDATION(S): Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD), to execute, on behalf of the County, Contract Amendment Agreement #22–986–1 with Monument Community Partnership, a non-profit corporation, effective October 1, 2009, to amend Contract #22–986, to increase the payment limit by $67,500, from $118,001 to a new payment limit of $185,501, with no change in the original term of July 1, 2009 through June 30, 2010. FISCAL IMPACT: This Contract is funded 64% by California Nutrition Network and 36% by the Hospital Council of Northern and Central California.
