RECOMMENDATION(S):
ACCEPT presentation from Contra Costa Health Services on Introduction and Implementation of Lean
Methodology for system improvement.
FISCAL IMPACT:
None.
BACKGROUND:
CCHS was chosen by the California Association of Public Hospitals – Safety Net Institute to use Lean
Methodology improve performance and patients’ experience throughout the system of care. This presentation
provides an overview of ongoing efforts at Contra Costa Regional Medical Center and initial results to date.
1. CCRMC started an effort to redesign its systems to increase patient safety, identify improvement opportunities
and ways to enhance the patient experience in 2005.
2. In the fall of 2008, CCRMC was one of four health systems in the state awarded a grant from the Safety Net
Institute to use Lean Management methodology to improve key performance indicators.

3. The key indicator chosen for the rollout was congestive heart failure readmission rates.
4. An executive intensive leadership training followed by four week-long Lean events involved teams across the entire
system, focusing on improvements that would enhance patient experiences and improve outcomes.
5. This presentation will describe how staff at all levels, and users of CCRMC services are involved in the Lean
process and what results were accomplished. It will highlight a new innovative effort to incorporate patients and
families in improvement efforts.
