Approve and authorize the Health Services Director, or his designee (Wendel Brunner,
M.D.) to sign a Application and Permit for Use of School Property with Oakley Union
Elementary School District for use by the Health Services Department's Public Health
Division Immunization Program for flu vaccine clinics including to hold harmless the
District for claims arising out of use of the facility for the sites and dates below.

BACKGROUND: (CONT'D)
Program will administer flu vaccine at elementary schools in Contra Costa County where
50% or more of the children are eligible for free/reduced lunch. Approval of this
Application and Permit for Use of School Property will allow the County to provide flu
vaccines at Oakley Union Elementary Schools, through December 31, 2009.
