Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #26-935-11 with David Kleinerman, M.D.,
a self-employed individual, effective July 1, 2009, to amend Contract #26-935-10 to
increase the payment limit by $5,500, from $1,025,250 to a new payment limit of
$1,030,750, with no change in the original term of August 1, 2008 through July 31, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I.
BACKGROUND:
On August 5, 2008, the Board of Supervisors approved Contract #26-935-10 with David
Kleinerman, M.D., for the period from August 1, 2008 through July 31, 2011, for the
provision of urology services including clinic coverage, consultation, training, medical
and/or surgical procedures and on-call services at Contra Costa Regional Medical Center.
At the time of negotiations the Payment Limit was based

on target levels of utilization. However, the utilization during 2008/2009 term of the
agreement was higher than originally anticipated. Approval of Contract Amendment
Agreement #26-935-11 will allow the Contractor to provide additional anesthesiology
services at Contra Costa Regional Medical Center and Contra Costa Health Centers,
through July 31, 2011.
