Approve and authorize the Health Services Director, or his designee (Patricia Tanquary), to
execute, on behalf of the County, Contract Amendment/Extension Agreement #27-277-11
with Kaiser Foundation Health Plan, Inc., a non-profit corporation, effective September 30,
2009, to amend Contract #27-277-8 (as amended by Amendment Agreement #27-277-9 and
#27-277-10), to increase the Contract payment limit by $3,600,000, from $36,900,000, to a
new total of $40,500,000, and extend the Contract term from September 30, 2009, through
December 31, 2009.
FISCAL IMPACT:
This Contract is funded 100% by Contra Costa Health Plan member premiums. Costs
depend upon utilization. As appropriate, patients and/or third-party payors will be billed for
services.
BACKGROUND:
On October 3, 2006, the Board of Supervisors approved Contract #27-277-8 (as amended
by Amendment Agreement #27-277-9 and #27-277-10) with Kaiser Foundation Health
Plan, Inc., for the period from October 1, 2005 through September 30, 2009, for the

BACKGROUND: (CONT'D)
health care services for Medi-Cal recipients enrolled in Kaiser Foundation Health Plan.
County and Contractor have agreed to extend the term for an additional year at the current
capitation rates in order to provide County with pricing stability for services provided to
Medi-Cal recipients. Approval of Contract Amendment/Extension Agreement #27–277–11
will allow the Contractor to continue providing services through December 31, 2009.
