Approve and authorize the Health Services Director, or his designee, to execute, on behalf
of the County, Contract Amendment Agreement #26-535-3 with Nazia R. Choudhury,
M.D., a self employed individual, effective August 1, 2009, to amend Contract #26-535-1
(as amended by Amendment Agreement #26-535-2) to decrease the payment limit by
$466,667, from $948,313 to a new payment limit of $481,646, with no change in the
original term of July 1, 2008 through June 30, 2011.
FISCAL IMPACT:
This Contract is funded 100% by Enterprise Fund I.
BACKGROUND:
On June 24, 2008, the Board of Supervisors approved Contract #26-535-1 (as amended by
Amendment Agreement #26-535-2) with Nazia R. Choudhury, M.D. for the period from
July 1, 2008 through June 30, 2011, for the provision of anesthesiology services including
consultation, training, medical procedures, on-call and clinical coverage.

BACKGROUND: (CONT'D)
At the time of negotiations the Payment Limit was based on target levels of utilization.
However, due a temporary leave of absence requested by Contractor and agreed upon by
County, utilization in 2009/2010 and 2010/2011 will be less than originally anticipated.
Approval of Contract Amendment Agreement #26-535-3 will allow the Contractor to
continue to provide anesthesiology services at Contra Costa Regional Medical Center and
Contra Costa Health Centers, through June 30, 2011.
