To meet its goal of increasing the quality of care provided to the County residents, CCHS is in need of a Chronic Disease Management System to manage all of its chronic disease patients and to facilitate accurate and efficient reporting, both internally and externally, set by the Health Resources and Services Administration (HRSA). After an RFP process the i2i Systems product i2iTracks was selected to fulfill this need. i2i Systems has a successful track record in the design, implementation, and support of chronic disease management systems and is able to exchange patient health data with the major systems installed at CCHS. A Chronic Disease management System (CDMS), also called a disease registry, is an electronic system used to capture, manage and provide information on specific conditions to support organized care management for patients This will enable Contra Costa Health Services (CCHS) physicians to provide population-based management of chronic diseases, as well as point-of-care electronic reminders of associated preventive screening and care for diabetes and other chronic diseases for our safety net patients. This system would make the clinician’s work easier because it provides real-time decision support and updated clinical information which allows for the implementation of routine care by other members of the health team while affording the provider quality time for direct patient care and other complex tasks. Moreover, the CDMS will enable CCHS to: Share appropriate clinical data with disease management and quality improvement collaboratives. Comply with payor, state, and federal reporting requirements. Streamline and standardize administrative and clinical processes. Provide access to complete and relevant patient information at all locations.
