Approve and authorize the Health Services Director, or his designee (Donna Wigand), to
execute, on behalf of the County, Novation Interagency Agreement #74–193–8 with Mount
Diablo Unified School District, a government agency, in an amount not to exceed $930,952,
to provide to provide wraparound services for Seriously Emotionally Disturbed (SED)
children and their families for the period from July 1, 2009 through June 30, 2010. This
Contract includes a six-month automatic extension through December 31, 2010, in an
amount not to exceed $465,476.
FISCAL IMPACT:
This Agreement is funded 50% by Federal FFP Medi-Cal, 45% by State Early and Periodic,
Screening, Diagnostic and Treatment Program (EPSDT), 4% by Mount Diablo Unified
School District and 1% by Mental Health Realignment.
BACKGROUND:
This Agreement meets the social needs of County’s population by providing child-family
team facilitators and other wraparound services to families of children with

BACKGROUND: (CONT'D)
serious emotional and behavioral disturbances; facilitates multi-agency collaborative service
delivery; and minimizes the need for crisis services and involvement with the Juvenile
Justice System.
On January 27, 2009, the Board of Supervisors approved Novation Interagency Agreement
#74–193–7 with Mount Diablo Unified School District, for the period from July 1, 2008
through 30, 2009, which included a six-month automatic extension through December 31,
2009, for the provision of wraparound services for SED children and their families.
Approval of Novation Interagency Agreement #74–193–8 replaces the automatic extension
under the prior Agreement and allows the Contractor to continue providing services through
June 30, 2010.
CHILDREN'S IMPACT STATEMENT:
This EPSDT program supports the following Board of Supervisors’ community outcomes:
“Children Ready For and Succeeding in School”; “Families that are Safe, Stable, and
Nurturing”; and “Communities that are Safe and Provide a High Quality of Life for
Children and Families”. Expected program outcomes include an increase in positive social
and emotional development as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS).
