Approve and authorize the Health Services Director, or his designee (Donna Wigand), to
execute, on behalf of the County, Contract Amendment Agreement #74–287–4 with
FamilesFirst, Inc., a non-profit corporation, effective June 1, 2009, to amend Novation
Contract #74–287–3, to increase the payment limit by $48,458, from $215,509 to a new
payment limit of $263,967, with no change in the original term of July 1, 2008 through June
30, 2009, and to increase the automatic extension payment limit by $24,229, from $107,755
to a new payment limit of $131,984, with no change in the term of the automatic extension,
through December 31, 2009.
FISCAL IMPACT:
This Contract is funded 18% by Federal FFP Medi-Cal, 16% by State Early and Periodic
Screening, Diagnosis, and Treatment (EPSDT), 23% by the Mount Diablo Unified School
District, and 43% by Individuals with Disabilities Education Act (IDEA/SB 90).

BACKGROUND: (CONT'D)
#74–287–3 with FamilesFirst, Inc. for the period from July 1, 2008 through June 30, 2009,
which included a six-month automatic extension through December 31, 2009, for the
provision of school based mental health services for seriously emotionally disturbed
students in the Mt. Diablo Unified School District and their families.
Approval of Contract Amendment Agreement #74–287–4 will allow the Contractor to
provide services to additional children, through June 30, 2009.
