APPROVE and AUTHORIZE the Employment and Human Services Director, or designee,
to execute a contract with Rehabilitation Services of Northern California in an amount up to
$196,410 to provide Linkages and Linkages Respite Program services to prevent the
premature or inappropriate institutionalization of frail, at risk elderly and functionally
impaired adults ages 18 years and older, for the period of July 1, 2009 through June 30,
2010
FISCAL IMPACT:
$196,410 100% State, Older Californians Act
BACKGROUND:
The intent of the Linkages Program is to prevent premature or inappropriate
institutionalization of frail, at risk elderly and functionally impaired adults, age 18 years and
older, by providing case management services as well as comprehensive information and
assistance services. The Linkages Program is designed to be a "gap filler' which assists
individuals at risk of institutionalization. Linkages case managers will use the client's
informal support system and existing community services to the fullest capacity in assisting

BACKGROUND: (CONT'D)
The Linkages Respite services are a componenet of Linkages case manangement provided
to directly make avialabler respite care to primary caregivers of Linkages clients when
finances are limited and the person providing the care is not eleigibler for or receiving
respite services from other spurces. The services may include in-home respite care, day
care, or 24-hour care for the respite period.
In addition to client services, the Linkages sites are responsible for participating in ongoing
community development activities related to community-based long-term care.
