SPECIFIC REQUEST(S)- OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS:
1 . Request the Health Services Director to draw up a proposal
and submit it to the Board of Supervisors for approval and
submission to the State Department of Health Services. The
proposal should be designed to demonstrate that more
appropriate care can be provided to low-income senior
citizens at a savings to the Medi-Cal and Medicare program
by using Medi-Cal and Medicare funds to supplement payments
made by an individual to a residential care facility thereby
reducing the need for unnecessary nursing home admissions.
The Medi-Cal and Medicare programs would save the difference
between the supplemental payment to the residential care
facility and the amount that would otherwise be paid to a
nursing home.
2 . In order to properly prepare such a demonstration project
proposal, request the Health Services Director to determine
the number of Medi-Cal and Medicare patients now in nursing
homes in Contra Costa County who medically could be cared
for in a residential care facility, how much of a
supplemental payment would have to be made on top of what
the patient can afford to pay to make such a residential
care facility a viable business venture, and what the
corresponding savings might be projected to be for the
Medi-Cal and Medicare programs.
3 . Remove this item as a referral to our Committee.
BACKGROUND:
In November of 1984, at the request of Supervisor Fanden, the
Board referred to our Committee a request to determine the need
for residential care facilities for the low-income elderly in
Contra Costa County. The concern expressed at that time was that
many middle income senior citizens who are no longer physically
able to maintain their own homes can live in retirement homes, or
residential board and care homes, as long as they have funds to
pay the fees which generally run from $800 to $1200, or more,

per month. These facilities and, therefore, this type of care is
unavailable to low-income senior citizens who are receiving
Supplemental Security Income (SSI ) , the maximum payment for which
is currently $589 . 00. The result is that senior citizens who
have only SSI to live on must either live in inadequate rental
situations which may actually be dangerous because of the
patient' s physical condition, or be admitted to nursing homes
where Medi-Cal and Medicare pays upwards of $2000 a month for a
patient who could be cared for elsewhere if there were only a few
hundred dollars a month supplementation to the patient' s SSI
income.
Our Committee met on December 9, 1985 with Julie Peck, Director
of Geriatric Services for the Health Services Department. Ms.
Peck shared with our Committee the attached November 14, 1985
report which documents the need for some relief to this
situation. Our inability to provide the most appropriate level
of care for these low-income senior citizens means that nursing
home beds are unnecessarily taken up by patients who don' t have
to be there, at greater cost to the Medi-Cal and Medicare
programs. This situation, in turn, contributes to a shortage of
nursing home beds, forcing some patients to remain in acute
hospitals when they could be cared for in a nursing home if beds
were available. This additional care in an acute hospital costs
the Medi-Cal and Medicare programs even more money.
Increasingly, Medi-Cal and Medicare are refusing to pay these
costs and often the county ends up paying for the care until a
nursing home bed becomes available.
Our Committee believes that in order to relieve pressure on the
whole continuum of care it is necessary to make more residential
care beds available to low-income senior citizens. We are,
therefore, recommending that the Health Services Department
prepare a proposal which would allow supplementation of a
patient' s SSI income from Medi-Cal ,and Medicare funds in order to
make residential care an available option for some nursing homes.
We believe that a proposal can be prepared which will demonstrate
the potential for savings of money throughout the system and
which will provide more appropriate care for the patients
involved. It may even be that the County would want to propose
converting one wing of the Merrithew Hospital to residential care
if the proposal can be shown to be financially viable and
sufficient residential care beds are not available. Or, a
cooperative plan might be developed with a private, nonprofit
organization to operate such a facility for, or in cooperation
with, the county.

You requested a report, on behalf of Supervisor Nancy Fanden, describing
the various types of -facilities in Contra Costa County available to
low-income senior citizens who are unable to live on their own but do not
require skilled nursing services.
There is a level of care between independent living and institutionalization;
this level of care is referred to as "residential care". The type of
facility licensed under residential care is a board and care home, which
typically is 'a private home, licensed by the State Department of Social
Services for a maximum occupancy of 6 adults. Supervision, as well as three
meals a day, is provided at this level'6f care. In order to live in a
residential care facility (board and care home) , an older person has to be
ambulatory, continent., and able to manage his/her ADL (activities of daily
living) with minimal assistance.
The attached report (Sept; 1985) lists the residential care facilities in
Contra Costa County. According to this report, there are 119 licensed
residential care homes for the elderly, persons 62 years and older. The
average occupancy rate for residential care is 6 persons; however, in
Contra Costathereare several large facilities licensed for higher
occupancy, e.g. Pleasant Hill RetirementHome. - 44, Famiiy-.Affair (Concord)
at 125, and Concord Royale - 180.
The cost to reside is a residential care facility in Contra Costa County
ranges .from .$500.00 a month to $1950.00 a month. The rates are set by
the facility operators/owners.
An issue that Supervisor Nancy Fanden is concerned about is the lack of
residential care options for elders on fixed, limited incomes. This is
indeed a problem. The majority of residential care homes in West Contra
Costa County will take elders who are on SSI (Supplemental Security Income) ,
the maximum of which is currently $589.00 a month. (To be eligible for
SSI, an elder must have an income of less than and total
savings of less than $1600.00) . There are some homes in East Contra Costa
Couhy that will also accept elders at the SSI rate. However, there are few
homes in Central Contra Costa County that will accept the SSI rate, and
Central Contra Costa County has the higher percentage of elderly residents,
and it is the area that most elders wish to remain in residence.

In the Past -few months, Geriatric Services' staff visited 57 residential
care homes •.in Central. and East County. Out of. the 21 homes visited in
East County, only 7 will accept the SSI rate. The R.K.M. Management. Company
has purchased the .old,lLos: Medanos Hotel site in Pittsburg and is in the
process'- of .building a 100-room facility which will have the capacity for
200 elders. This company has .made a commitment- to serving SSI recipients
and .plans .to have a 75% SSI occupancy level. This facility, when completed,
will certainly enhance residential care options for elders in East County
and, of course, will be an available option for elders living in other
areas of Contra Costa County.
Out of the .36 homes visited.in Central:.County, only 2 will accept the SSI
rate. The average cost in Central County fora shared room is $750.00 per
month; $900 .per month for a private room. In .many instances, families are
forced: to subsidize their parent's monthly income (usually by several hundred
dollars per month) .in order for their parent to reside in a residential care
facility (board- and care home) in Central County.
The vacancy factor .for residential care in Central County is very, very
low, and itis often difficult to find an opening in a desirable residential
home in; this part-of the County. The vacancy factor is higher in East County
and considerably higher in West County.
Attached you will find a .3 x 5 information card on the 57 residential care
facilities visited by my staff. These -cards outline basic information about
each facility (address, phone number, occupancy, rates, etc.. ) and, also on
the back of each card, have subjective information about each home. Please
note that the subjective information is based on the. Geriatric Services' staff
observations and feelings about each facility visited. I hope this information
will be useful to- Supervisor Nancy Fanden .and her staff.
