Approve and authorize the Health Services Director, or his designee (Donna Wigand) to
execute on behalf of the County, Contract Amendment Agreement #74-271-82(2) with Lori
Salinas, MFT, a self-employed individual, effective June 1, 2009, to amend Contract
#74-271-82 [as amended by Contract Amendment Agreement #74-271-82(1)] to increase
the payment limit by $40,000, from $80,000 to a new payment limit of $120,000, with no
change in the original term of July 1, 2008 through June 30, 2010.
FISCAL IMPACT:
This Contract is funded 100% by Medi-Cal Funds, offset 50% State and 50% Federal.
BACKGROUND:
In June 2008, the County Administrator approved and the Purchasing Services Manager
executed Contract #74-271-82 [as amended by Contract Amendment Agreement
#74-271-82(1)] with Lori Salinas, MFT, for the period from July 1, 2008 through June 30,
2010, to provide Medi-Cal specialty mental health services. At the time of negotiations, the
payment limit was based on target
