HomeMy WebLinkAboutMINUTES - 06232009 - C.16RECOMMENDATION(S): APPROVE the attached report for the Relief of Shortage and Account Collection involving a $7,500 life insurance refund and AUTHORIZE the referral to the County Office of Revenue Collections for recovery of funds. FISCAL IMPACT: $8,467.23 ($7,500 life insurance policy plus interest) repayment to the ING ReliaStar Life Insurance Company BACKGROUND: On April 27, 2007, the Contra Costa County Employee Benefits Services Unit authorized payment of a life insurance benefit in the amount of $7,500 for a deceased employee. The claim was processed as payable to the spouse of the deceased employee because no filed designation of beneficiary was found. Later it was determined that proper designation of beneficiary for the $7,500 was the mother of the deceased employee.
